Automated Case Creation: Automatically create cases in Monarch based on triggering events in your EHR.
Groups: Use groups to manage access to cases and assign for review.
Routing: Route cases between groups for additional review, while choosing what information to share.
Escalation Policies: Define policies that automatically route cases through your health system's patient safety structure when certain conditions are encountered.
Lineage: View the history of how an event was routed and escalated through your organization.
Labels: Add labels to help categorize cases and use labels as filters in reports.
Document Upload: Upload Documents to your Cases (e.g., relevant literature, M&M presentation, or hospital policies).
Classification: Classify event severity with Monarch's built-in classification system, or supply your own (e.g., Clavien-Dindo).
Conversations: Discuss a case with another clinician through a secure, email-like interface.

Events are commonly the result of a system or process failure, rather than the actions of an individual. Monarch's Taxonomy of Failure Modes can help you better understand how events occur and identify root cause.

Complications & Outcomes: Track procedure and provider-level complication and outcome data. Customize these values to suit your needs or use Monarch-supplied values.
Failure Mode Analysis: Build a high-reliability environment by uncovering and documenting System & Individual Failure Modes.

Improve Outcomes

Once root cause is identified, changes must be implemented to prevent future recurrence. Action Items are assignable, trackable tasks tied directly to the originating case.

Action Items: Create action items to implement and monitor change.
Assignees: Assign action items to individuals to hold them accountable.
Follow-Up: Send follow-up emails to assignees from inside Monarch.
Email Notifications: Assignees are notified via email upon assignment.
Reminders: The system will automatically remind assignees when an action item is nearing its due date. Reminders are customizable.
